This is my favorite application question ever:

1. This position is 2 miles from your home address. Are you willing to travel this distance to work?

No. No, I'm not. If I have to back out of my driveway to get there, it is too damn far.

Seriously. You have to wonder if the company you're applying to has had issues with it before though. Why else would they ask it? If someone said "2 miles is way too far" I'd laugh in their face!
